宫腹腔镜联合手术治疗子宫瘢痕妊娠的临床研究

【摘要】 目的：探討子宫瘢痕妊娠患者经过腹腔镜联合宫腔镜治疗的临床效果。方法：选择2015年1月-2017年12月江西省妇幼保健院治疗的子宫瘢痕妊娠患者60例的资料进行回顾性分析。根据患者的治疗方法分为试验组（n=30）和对照组（n=30）。试验组采用腹腔镜联合宫腔镜微创手术治疗，对照组采用甲氨蝶呤化疗加清宫治疗，分析两组患者治疗效果。结果：对照组住院时间长于试验组，术后再次月经出现时间晚于试验组，手术出血量多于试验组，差异均有统计学意义（t=-6.052、-3.621、-5.168，P<0.05）;对照组术后出现细菌感染比例高于试验组，一次性根治患者比例低于试验组，差异均有统计学意义（字2=4.706、10.417，P<0.05）。结论：腹腔镜联合宫腔镜治疗子宫瘢痕妊娠，有住院时间短、出血量少、术后感染率低、一次性根治率高、术后再次月经出现时间快等优点。

【Abstract】 Objective：To investigate the clinical effect of laparoscopy combined with hysteroscopy in patients with uterine scar pregnancy.Method：The data of 60 cases of uterine scar pregnancy treated by Jiangxi Maternal and Child Health Hospital from January 2015 to December 2017 were analyzed retrospectively.According to the treatment method of the patients，they were divided into the experimental group（n=30） and the control group（n=30）.The experimental group was treated with laparoscopy combined with hysteroscopic minimally invasive surgery，the control group was treated with Methotrexate plus uterine cure for uterine incision pregnancy.The therapeutic effect of the two groups were analyzed.Result：The hospitalization time in the control group was longer than that in the experimental group，and postoperative menstruation time was later than that in the experimental group，and the amount of bleeding in the operation was more than that in the experimental group，the differences were statistically significant（t=-6.052，-3.621，-5.168，P<0.05）.The proportion of bacterial infection in the control group was higher than that in the experimental group，the proportion of patients who underwent radical cure was lower than that in the experimental group，the differences were statistically significant（字2=4.706，10.417，P<0.05）.Conclusion：Laparoscopy combined with hysteroscopy in the treatment of uterine scar pregnancy has the advantages of short hospital stay，less bleeding，low postoperative infection rate，high one-off radical treatment rate and fast postoperative menstruation.

因此探讨诊治子宫瘢痕妊娠的安全有效方法，改善患者预后是目前首先要解决的问题。临床已有的研究分析出宫腹腔镜联合治疗具有明显的治疗优势：腹腔镜下观察可将实物放大2～4倍，利用气腹减少出血更可以将盆腔视野清晰地展现出来，以对盆腔进行全面观察，准确地评估盆腔脏器形态结构、病变制定合理的治疗方案[16-17]。Pirtea等[18]综合目前的文献表明腹腔镜方法与异位妊娠切除是最好的选择。Fu[19]回顾性研究了205例CSP患者，其中189例手术治疗。其中宫腔镜刮治111例，认为宫腔镜检查提供了一个清晰的视野，使我们可以很容易地确定受影响的组织范围，并清洁妊娠组织。超声刮除术是一种相对简单、廉价的方法。Glenn等[20]综合了最近的研究表明支持任何消除妊娠和瘢痕的方法，以减少发病率并促进未来的生育能力。腹腔镜和经阴道方法是CSP治疗的选择，尽管需要继续研究以确定最佳方法。

本研究中试验组利用宫腹腔镜联合治疗子宫瘢痕妊娠，对照组采用MTX化疗加清宫治疗子宫瘢痕妊娠。通过比较二组患者的住院时间、术中出血、术后感染、一次性治疗成功率均证实宫腹腔镜联合治疗子宫瘢痕妊娠是一种创伤小、安全性高、治疗效果确切、依从性好、能最大限度保留生育功能的治疗方法，值得临床进一步深入研究探讨。
