糖尿病患者血浆中半乳糖凝集素-3水平的表达

目的 探讨半乳糖凝集素-3(Galectin-3)在糖尿病患者血浆中的表达情况，以及与其他因素的相关性分析。方法 对30例糖尿病患者、22例非糖尿病患者(对照组)血浆中的Galectin-3进行测定，比较两组间Galectin-3水平变化，分析Galectin-3与其他因素的相关性。结果 糖尿病患者血浆中的Galectin-3水平[(17.95±2.61) μg/L]显著高于对照组[(15.59±2.39)μg/L]，差异有统计学意义(P<0.01)。Galectin-3与空腹血糖(FBG)有显著地相关性(P<0.01)，与糖尿病病程(DD)、钠尿肽(BNP)有明显相关性(P<0.05)。结论 Galectin-3作为重要的炎症调节因子，可能在糖尿病的发生发展中起着重要的调节作用。

近年研究发现，半乳糖凝集素(Galectin-3)是一种重要的炎症、免疫、肿瘤调节因子，在心力衰竭的诊断、预后评估中具有重要价值[1-2]。2010年，FDA批准了Galectin-3的临床应用，目前Galectin-3已成为欧美国家临床诊断与评估心力衰竭患者近期预后的重要指标[3-4]。然而，作为重要的炎症调节因子，Galectin-3在糖尿病及其并发症中的作用研究还比较罕见。本文就Galectin-3在糖尿病患者血浆中的表达水平，以及与其相关的因素进行了初步探讨。

1 对象与方法

1.1 研究对象 选取2013年1—5月在我院老年心血管科住院的患者52例，其中糖尿病患者30例为研究组，男28例，女2例，年龄48～87岁，平均年龄(65.3±9.7)岁；非糖尿病患者22例为对照组，男19例，女3例，年龄48～86岁，平均(61.6±12.1)岁。入选对象排除标准：心力衰竭患者、肿瘤患者、严重肝肾功能不全患者。两组总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、肌酐(Cr)、尿素氮(BUN)和尿酸(BUA)水平，以及高血压、冠心病患病率两组间差异均无统计学意义(P>0.05)。

1.2 方法 Galectin-3检测，入院次日清晨空腹采静脉血4 mL于EDTA抗凝管中，在4℃状态下离心3000 r/min ,20 min，分离血浆放置-80℃保存，择日统一由北京海斯特医学检验中心测定，试剂为BG medicine 公司的Galectin-3 Elisa检测试剂盒。钠尿肽(BNP)、各项生化指标由我院检验科测定。

1.3 统计学处理 采用SPSS 17.0统计软件，计量资料组间比较用独立样本的t检验；计数资料采用χ2检验；相关性分析采用Pearson相关分析。P<0.05为差异有统计学意义。

2 结果

2.1 两组患者血浆中Galectin-3水平的比较 与非糖尿病组比较，糖尿病组Galectin-3水平显著增高，差异有统计学意义(P<0.01)；BNP水平两组间差异无统计学意义(P>0.05)，见表1。

2.2 Galectin-3与部分临床变量的相关性分析 见表2。

3 讨论

糖尿病的发病机制被认为与机体的炎症过程密切相关，已形成糖尿病的炎症发病学说。国外研究提示，Galectin-3通过促进炎症的发生，来加速代谢性疾病的发病机制[5-6]。因此，作为炎性反应重要介质的Galectin-3，在糖尿病及其并发症中的作用必然受到人们的关注[7]。Seferovic等[8]研究发现，糖尿病合并高血压患者血浆中的Galectin-3水平显著高于单纯糖尿病或高血压患者。Jin等[9]研究结果显示，2型糖尿病伴血管并发症患者血浆中的Galectin-3水平明显高于无并发症的糖尿病患者(P<0.05)，Galectin-3水平升高可增加2型糖尿病患者血管并发症发生的风险；Galectin-3与糖尿病病程、糖化血红蛋白有明显的相关性(P<0.05)。作者认为，Galectin-3与糖尿病患者血管病的发病机制有关。目前，我们尚未见到研究糖尿病与非糖尿病患者之间Galectin-3水平变化及其作用的文献。本文研究结果显示，糖尿病患者血浆中的Galectin-3水平显著高于非糖尿病者(P<0.01)，而同样作为心力衰竭诊断标志物的BNP则无明显变化。而且，经Pearson相关性分析表明，Galectin-3水平与糖尿病患者的病程、FBG水平和BNP有显著的相关性(P<0.01或P<0.05)。本文研究结果提示，Galectin-3不仅是一种心力衰竭的诊断标志物，而且还是一种炎症调节因子参与炎症的发生发展。FBG水平越高，糖尿病病程越长，血浆中的Galectin-3水平则表达的越高，提示炎症程度越高，病情越严重，发生糖尿病并发症的风险越大。

Expression of plasma galectin-3 in patients with diabetes mellitus

Objective To analyze the plasma galectin-3 level in patients with diabetes mellitus and its relevant risk factors.Methods Thirty patients with diabetes mellitus served as study group and twenty-two individuals without diabetes mellitus served as control group were enrolled in this study.The levels of plasma Galectin-3 were measured and compared between the two groups.The correlation between plasma galectin-3 and other risk factors were analyzed.Results Levels of plasma Galectin-3 was (17.95±2.61)μg/L in study group,and (15.59±2.39)μg/L in control group.Difference in levels of plasma galectin-3 between the two groups was significant(P<0.01).Pearson analysis showed that plasma galectin-3 had significant correlation with fasting blood glucose,duration of diabetes,and B-type natriuretic peptide(P<0.01 orP<0.05).Conclusion As an important regulation factor of inflammation,galectin-3 may play a role in the pathogenesis of diabetes mellitus.
