血清胱抑素C与2型糖尿病肾病的相关性研究

[摘要] 目的 探討2型糖尿病肾病患者肾功能、尿蛋白及代谢指标与血清胱抑素C水平的相关性。方法 选取2017年1—12月收治的244例2型糖尿病肾病患者为研究对象,将研究对象分为早期肾病组、临床肾病组、终末期肾病组。检测3组患者空腹血糖(FBG)、空腹C肽、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、甘油三酯(TG)、胆固醇(TC)、尿酸(UA)、尿素(BUN)、肌酐(SCr)、血胱抑素C(CysC)、尿白蛋白肌酐比值(UACR),并进行统计学分析。结果 3组间体重指数、TG、HDL水平比较差异无统计学意义(P>0.05),终末期肾病组年龄、血压、病程、TC、LDL、空腹C肽水平高于早期肾病组,且差异有统计学意义(P<0.05),临床肾病组、终末期肾病组UA、BUN、SCr、CysC、UACR水平均高于早期肾病组,且差异有统计学意义(P<0.05)。Spearman相关性分析表明CysC与2型糖尿病肾病患者年龄、病程、空腹C肽、UA、BUN、SCr、CysC、UACR呈正相关(P<0.05)。结论 CysC与DN进展具有相关性,有助于DN的早期诊断。

Study on the Relationship between Serum Cystatin C and Type 2 Diabetic Nephropathy

[Abstract] Objective To investigate the correlation between renal function, urinary protein and metabolic parameters and serum cystatin C level in patients with type 2 diabetic nephropathy. Methods A total of 244 patients with type 2 diabetic nephropathy admitted to our hospital from January to December 2017 were selected as subjects of study. The subjects were divided into early nephropathy group, clinical nephropathy group and end-stage nephropathy group. Fasting blood glucose (FBG), fasting C-peptide, high density lipoprotein (HDL), low density lipoprotein(LDL), triglyceride(TG), cholesterol (TC), uric acid (UA), urea (BUN), creatinine (SCr), cystatin C(CysC), and creatinine ratio of urine albumin (UACR) were measured and analyzed statistically. Results There was no significant difference in body mass index, TG and HDL levels among the three groups(P>0.05). The levels of age, blood pressure, course of disease, TC, LDL and fasting C-peptide in end-stage nephropathy group were higher than those in early-stage nephropathy group, and the difference was statistically significant(P<0.05). The levels of UA, BUN, SCr, CysC and UACR in clinical nephropathy group and end-stage nephropathy group were higher than those in early nephropathy group, and the difference was statistically significant(P<0.05). Spearman correlation analysis showed that CysC was positively correlated with age, course of disease, fasting C-peptide, UA, BUN, SCr, CysC and UACR in patients with type 2 diabetic nephropathy(P<0.05). Conclusion CysC is correlated with DN progress, which is helpful for early diagnosis of DN.
