卵泡液中细胞因子与卵泡发育及妊娠结局的相关性

卵泡液中细胞因子与卵泡发育及妊娠结局的相关性

【摘要】随着体外受精-胚胎移植(in vitro fertilization-embryo transfer,IVF-ET)技术水平的日益提高,卵巢微环境中调节卵母细胞发育的细胞因子的作用受到广泛关注。细胞因子作为一类具有广泛生物学活性的激素样多肽调控生殖过程,它们通过调节卵巢内分泌或旁分泌,进而影响卵泡成熟、胚胎发育及胚泡着床等环节来参与生殖调节。研究表明不孕可能与卵泡液中某些细胞因子(如血管内皮生长因子、粒细胞集落刺激因子、活性氧簇、胰岛素样生长因子、白细胞介素、白血病抑制因子等)的异常表达引发的卵泡成熟障碍有关。因而,加强卵泡液中细胞因子的研究将有助于提高IVF-ET的临床妊娠率。

女性不孕的病因是现代生殖医学研究的热点之一。多项研究发现,卵泡液中细胞因子表达异常与卵泡发育及排卵障碍有关,现就近年发表的有关卵泡液中细胞因子表达水平与卵泡发育及辅助生殖临床结局相关的文献进行综述。

1 血管内皮生长因子(vascular endothelial growth factor,VEGF)和内分泌腺来源的VEGF(endocrine gland-derived VEGF,EG-VEGF)

VEGF是一种肝素结合双链糖蛋白,能特异地促进血管内皮增生。正常妇女的VEGF在卵泡颗粒细胞和卵泡膜细胞呈周期性表达,在始基卵泡和闭锁卵泡中无表达,随着卵泡发育表达增加,促性腺激素和低氧也可刺激其表达,在促进卵巢血管生成、维持卵巢功能方面起重要作用,且能够促进排卵前卵泡向血管化黄体的转变,是与着床和早期妊娠相关的重要生血管因子[1]。Babitha等[2]发现水牛卵泡液中的VEGF可通过促进卵巢组织毛细血管增生促进卵泡选择、生长及成熟。金锐等[3]的研究结果显示,在促排卵周期随着卵泡生长,血清中VEGF水平呈上升趋势,但获卵数≥20个组的患者卵泡液中VEGF水平低于获卵数≤5个组,认为成熟卵泡数目增加可能会抑制单个卵泡中VEGF的生成;而卵巢低反应者卵泡周围微循环不足,缺氧导致红细胞代偿性增多,可促进VEGF mRNA的合成。

EG-VEGF是一种组织特异性血管生成因子,作为内分泌腺微血管床内皮细胞的有丝分裂原,调节内皮细胞的生长、结构和功能,诱导内分泌腺中新血管形成。卵巢是人体EG-VEGF合成量最高的器官,主要由卵泡的颗粒细胞和膜细胞表达。有学者证实,卵泡液中VEGF和EG-VEGF浓度与卵巢反应性及卵母细胞成熟度呈负相关,血清EG-VEGF的浓度与妊娠结局相关,认为由于卵巢颗粒细胞和膜细胞是人体合成EG-VEGF的主要细胞,EG-VEGF在卵泡发育中的作用可能较VEGF更为重要,EGVEGF可通过提高子宫内膜容受性改善妊娠结局[4-5]。另有学者发现体外受精(IVF)患者VEGF的基因型(+405G>C型)与胚胎种植率相关[6]。

VEGF和EG-VEGF的组织细胞定位和时空表达呈互补关系[7]。EG-VEGF无论在卵泡液还是血清中的浓度均与人绒毛膜促性腺激素(hCG)注射日血清雌二醇(E2)浓度呈负相关,而VEGF仅在卵泡液中的浓度与hCG注射日血清E2浓度呈负相关,提示EGVEGF与卵巢反应性关系更加密切。黄体早期血清中而非卵泡液中EG-VEGF水平与IVF妊娠结局有关,认为血清EG-VEGF浓度可能与子宫内膜容受性有关,EG-VEGF可能通过调节子宫内膜血管通透性和促进低密度脂蛋白转运入黄体细胞合成孕酮支持妊娠[8]。

2 粒细胞集落刺激因子(granulocyte colony stimulating factor,G-CSF)

G-CSF属于造血生长因子,主要由造血细胞产生,一些非造血细胞也可以产生G-CSF。人卵巢中的G-CSF主要由颗粒细胞、卵泡膜细胞和基底细胞产生,并通过自分泌或旁分泌发挥其生物学效能,与卵巢的微环境、储备功能及卵母细胞质量密切相关,并可作为预测妊娠结局的有效指标之一[9]。董毅飞等[10]发现IVF患者取卵日血清中G-CSF浓度与卵泡液中G-CSF浓度呈正相关,但明显低于卵泡液;妊娠组卵泡液中G-CSF浓度显著高于未妊娠组,且与获卵数呈正相关,而两组间血清G-CSF浓度差异无统计学意义。在IVF/胞浆内单精子注射(ICSI)周期中联合检测卵泡液中G-CSF、白细胞介素33(IL-33)及其受体IL -1受体样1(IL -1 receptor -like 1, IL1RL1,也称为ST2)的浓度可预测IVF的成功率[11]。Lédée等[12]也证实,卵泡液中G-CSF浓度与胚胎移植成功率呈正相关,能较准确地预测妊娠率。Barad 等[13]则认为卵泡液中G-CSF浓度与子宫内膜厚度、胚胎着床率及临床妊娠率无明显相关性,但该研究患者平均年龄>39岁,结论并不适用于年轻患者。

3 活性氧簇(reactive oxygen species,ROS)与总抗氧化能力(total antioxidant capacity,TAC)

ROS是有氧代谢的产物,在细胞凋亡、损伤修复和细胞增殖等信号传递途径中具有重要调控作用。过高水平的ROS会导致细胞损伤,机体抵抗ROS的能力称为TAC,若ROS-TAC动态平衡被打破,则可能导致细胞功能异常。卵泡液内ROS主要来源于巨噬细胞、颗粒细胞、上皮细胞以及卵母细胞的代谢,在卵泡发生和卵子发育等方面起重要调控作用[14],抗氧化酶则主要来自颗粒细胞和泡膜细胞。排卵前黄体生成激素(LH)峰的出现可引起多个炎症相关细胞因子表达增强,并出现局部急性炎症反应,巨噬细胞、中性粒细胞等释放大量的ROS。牛宏志等[15]发现,IVF患者取卵日血清ROS水平显著高于卵泡刺激前,认为排卵反应可导致卵泡液内ROS升高,并通过微循环进入血清中,导致血清ROS升高,而血清和卵泡液内TAC并未随排卵而升高,高水平的ROS虽不影响受精,但可能对胚胎的后续发育产生负面影响。Shkolnik等[16]的动物实验发现,注射hCG后卵泡内ROS明显升高,而使用大量抗氧化剂抑制ROS产生的同时排卵也受抑制,表明ROS水平与排卵密切相关。Singh等[17]发现,子宫内膜异位症及输卵管因素不孕患者中,妊娠组卵泡液中ROS水平低于非妊娠组,推测ROS水平可能与妊娠相关,但其结论及机制尚待进一步研究证实。总之,保持ROS-TAC水平的平衡可维持健康卵泡的活性,ROS-TAC失衡将对卵母细胞成熟和后续胚胎发育造成不良影响。

4 胰岛素样生长因子(insulin-like growth factor,IGF)

IGF是一种与胰岛素结构和功能相似的单链碱性蛋白,主要包括IGF-Ⅰ和IGF-Ⅱ。IGF-Ⅰ具有促进有丝分裂、参与新陈代谢等胰岛素样生物学效应,可以通过调节葡萄糖转运蛋白8(Glut-8)促进糖吸收,在增加胰岛素水平的同时引起IGF-Ⅰ升高,在生殖领域中发挥重要作用。多囊卵巢综合征(PCOS)患者高水平的胰岛素可通过IGF-Ⅰ影响卵泡生长[18]。Kafi等[19]发现,卵泡液中升高的孕激素、IGF-Ⅰ和高水平的E2可促进卵母细胞最后阶段的成熟。Mehta等[20]发现,卵泡液中IGF-Ⅰ水平较高的IVF患者受精率、卵裂率、囊胚形成率、胚胎种植率及临床妊娠率也显著升高。IGF-Ⅱ的mRNA及相应蛋白在优势卵泡的卵泡液及颗粒细胞中大量存在,而在PCOS患者卵泡及正常卵巢小窦状卵泡的卵泡膜细胞上弱表达,IGF-ⅡmRNA的表达与卵泡液中VEGF浓度呈正相关,并与卵泡生长及优势卵泡的成熟相关,妊娠患者血清及卵泡液中IGF-Ⅱ有高于未妊娠患者的趋势[21-23]。IGF家族成员与妊娠结局的相关性优于单一因子[24],但目前卵泡液中IGF表达与妊娠结局的相关性尚不明确。

5 IL和白血病抑制因子(leukemia inhibitory factor,LIF)

IL是一种在多种细胞中均有表达的糖蛋白,通过与其特异性受体结合传递信息,并影响生殖过程的各个环节,包括卵细胞发育、成熟、排卵以及受精、着床、妊娠等。IL-1β是IL-1系统具有生物活性的亚型之一,与生殖活动关系最为密切,卵泡液中IL-1β水平是反映卵母细胞质量及黄体功能的重要指标[25]。有研究表明,PCOS患者血清中IL-1β的高表达与PCOS发生有一定相关性,且患者血清中IL-6基因启动子区域的174 G/C单核苷酸多态性、基因型和等位基因分布与对照组间差异均有统计学意义,其中G等位基因频率与PCOS发病显著关联,卵泡液中IL-12表达水平与卵子受精及胚胎发育潜能呈正相关[26-28]。目前,炎症细胞因子IL参与PCOS发生的机制尚未明确,但IL与PCOS的关联性对PCOS的预防和治疗具有重要参考价值。

LIF是IL-6家族中的一个多效性细胞因子,是一种分泌型糖蛋白,LIF及其受体mRNA在卵泡发育不同阶段的颗粒细胞均有表达,随卵泡发育而增加,在排卵前达高峰,在闭锁卵泡表达消失,由此推测LIF可能参与排卵的局部调节。Haraguchi等[29]发现,猪体内存在LIF/信号转导和转录活化因子3(STAT3)通路,是否可通过补充LIF激活该通路提高卵母细胞的发育潜能尚待进一步研究证实。

综上所述,卵泡液内细胞因子的异常表达可影响卵泡的发育及成熟,但其机制尚不明确。随着高通量测序技术的发展,通过检测卵泡液中的相关细胞因子的基因表达,有望为评价卵子质量及促进卵泡发育成熟提供新的治疗策略。

Cytokines in Follicular Fluid were Related to Follicle Development and Pregnancy Outcomes

【Abstract】With the increasing technology in vitro fertilization-embryo transfer(IVF-ET), the roles of cytokines in the regulation of oocyte development in ovarian microenvironment have been widely concerned. Cytokines as a class of hormone-like peptides with comprehensive biological activity participate in the regulation of reproduction, such as the follicular development, embryo development and blastocyst implantation and others, by regulating ovarian hormones and paracrine. It was found that the abnormal expression of cytokine profile in follicular fluid, such as vascular endothelial growth factor(VEGF), granulocyte colony stimulating factor(G-CSF), reactive oxygen species(ROS), insulin-like growth factor(IGF), interleukin(IL), and leukemia inhibitory factor (LIF)could be related to the follicular mature disturbance. The enhancing research in this area will be helpful to improve the clinical pregnancy outcomes of IVF-ET.
