增强型体外反搏治疗冠心病伴心功能不全的疗效

摘 要 目的:探讨增强型体外反搏(EECP)治疗冠心病伴心功能不全的效果。方法:选取82例冠心病伴心功能不全患者,采用随机数字表法将其分为对照组和观察组各41例,对照组采用心脏康复训练,观察组采用心脏康复训练结合EECP治疗,比较两组患者的心功能指标、运动耐量和疗效。结果:治疗后观察组总有效率高于对照组;左心室射血分数(LVEF)高于对照组,左室收缩末内径(LVESD)、左室舒张末内径低于对照组;达到运动终点时间、6 min内最大步行距离高于对照组(P<0.05)。结论:心脏康复训练结合EECP治疗冠心病伴心功能不全能够有效改善患者的心功能,提高运动耐量和疗效。

Effect of EECP in the treatment of coronary heart disease with cardiac insufficiency

ABSTRACT Objective: To investigate the effect of the enhanced external counterpulsation (EECP) in the treatment of coronary heart disease with cardiac insufficiency. Methods: Eighty-two cases of the patients were selected and divided into a control group and an observation group with 41 cases each based on a random number table method. The control group underwent cardiac rehabilitation training while the observation group received cardiac rehabilitation training combined with EECP therapy. The indexes of cardiac function, the exercise tolerance and the curative effect were compared between the two groups. Results: After treatment, the total effective rate and the left ventricular ejection fraction (LVEF) were higher, the left ventricular endsystolic diameter (LVESD) and the left ventricular end-diastolic diameter were smaller and the time for reaching the end point and the maximum walking distance within 6 min were longer in the observation group than the control group (P<0.05). Conclusion: Cardiac rehabilitation training combined with EECP for the treatment of coronary heart disease with cardiac dysfunction can effectively improve the cardiac function and the exercise tolerance of patients and enhance the efficacy.

目前主要采用利尿、調脂、抗血小板聚集、硝酸酯类药物、β-受体阻滞剂等药物治疗冠心病[1-2]。相关报道显示,治疗期间进行心脏康复训练对改善预后有着积极的影响[3]。随着医学技术的不断发展,增强型体外反搏(enhanced external counterpulsation,EECP)作为一种无创、安全的机械循环辅助治疗技术,可有效增加机体心肌灌注,改善血管内皮功能,促进侧支循环建立,有益于冠心病病情的改善[4]。

1 资料与方法

1.1 一般资料

选取2017年3月—2019年3月佳木斯市中心医院收治的冠心病伴心功能不全患者82例,采用随机数字表法将其分为观察组和对照组各41例。其中观察组男23例,女18例;年龄31~75岁,平均年龄(62.49±6.62)岁;病程1~17年,平均病程(4.25±1.36)年。对照组男25例,女16例;年龄28~78岁,平均年龄(62.38±6.41)岁;病程1~16年,平均病程(4.38±1.17)年。两组患者一般资料比较差异无统计学意义(P>0.05),具可比性。
