鼻用糖皮质激素结合鼻腔冲洗治疗变应性鼻炎的疗效观察

摘要：目的 观察鼻用糖皮质激素结合鼻腔冲洗治疗变应性鼻炎的疗效。方法 回顾分析2015年11月～2016年11月在我院治疗的86例变应性鼻炎患者临床资料，将其随机分为对照组和观察组，每组43例患者。对照组患者采用鼻用糖皮质激素治疗，观察组患者在对照组治疗的基础上采用鼻腔冲洗治疗，观察对比两组患者临床治疗疗效。结果 观察组治疗有效率明显高于对照组，临床鼻部症状（鼻塞、鼻痒、喷嚏、流鼻涕）评分明显低于对照组，FENO值、RQLQ评分明显优于对照组，差异有统计学意义（P<0.05）。结论 鼻用糖皮质激素结合鼻腔冲洗治疗变应性鼻炎效果显著，可明显改善患者临床鼻部症状，提高患者生活质量，减轻气道炎症反应，值得临床推广和应用。

Abstract：Objective To observe the effect of nasal glucocorticoid combined with the effect of nasal douche in the treatment of allergic rhinitis. Methods A retrospective analysis from November 2015 to November 2016 in our hospital during the treatment of 86 cases of allergic rhinitis patients with clinical data， were randomly divided into control group and observation group， 43 patients in each group. The control group were treated by nasal treatment use of corticosteroids， patients in the observation group in the control group based on the treatment of the nasal irrigation treatment， observation and comparison of two groups of patients with clinical curative effect.Results The treatment efficiency of observation group was significantly higher than the control group， the clinical symptoms （nasal congestion， nasal itching， sneezing， runny nose） were significantly lower than the control group， FENO value. The RQLQ score was significantly better than the control group， the difference was statistically significant （P < 0.05）. Conclusion Nasal corticosteroids combined with nasal lavage for the treatment of allergic rhinitis effect significantly， can significantly improve clinical nasal patients Symptoms， improve the quality of life of patients， reduce airway inflammation， it is worth clinical promotion and application.

變应性鼻炎是一个公共卫生问题，临床治疗方法备受关注和重视。临床通常采用鼻用糖皮质激素治疗，对变应性鼻炎临床症状有一定的疗效，其全身生物利用度低，安全性良好[1]。该方法成为治疗变应性鼻炎的首要选择，但在临床中单一使用糖皮质激素对中-重度变应性鼻炎患者而言疗效大多都不满意。因此，临床寻找治疗疗效理想、且避免或减少不良反应少的治疗方法是当前迫切需要解决的问题之一。随着鼻腔冲洗在鼻腔、鼻窦炎等疾病治疗中的广泛应用，其操作简单，且具有良好的耐受性、安全性，是一种良好的治疗方法。本文作者结合2015年11月～2016年11月在我院治疗的86例变应性鼻炎患者临床资料，研究鼻用糖皮质激素结合鼻腔冲洗治疗变应性鼻炎的疗效，现报告如下。

1 资料与方法

1.1一般资料 回顾分析2015年11月～2016年11月在我院治疗的86例变应性鼻炎患者临床资料，将其随机分为对照组和观察组，每组43例患者。对照组43例患者中男性18例，女性25例；年龄19～50岁，平均年龄（34.19±4.22）岁；病程0.5～8年，平均病程（3.01±1.24）年。观察组43例患者中男性20例，女性23例；年龄18～50岁，平均年龄（35.0±3.56）岁；病程0.5～9年，平均病程（2.98±1.30）年。两组患者在年龄、性别、病程等基础资料方面差异无统计学意义（P>0.05）。

1.2纳入标准 符合《变应性鼻炎诊断和治疗指南》中关于变应性鼻炎的诊断要气；无过敏史者；使用丙酸氟替卡松鼻喷雾剂治疗效果不明显或治疗无效者；无急慢性上、下呼吸道感染；无酗酒、吸毒等不良习惯史；无鼻肿瘤、鼻外伤史、鼻中隔明显偏曲、鼻窦炎史。
