高频超声和SPECT对甲状腺癌诊断的联合应用研究

[摘要] 目的 探讨高频超声联合SPECT应用于甲状腺癌诊断中的临床价值。方法 方便选取2017年6月—2018年6月该院收治97例甲状腺肿瘤患者作为研究对象,全部患者均进行高频超声检查,并联合SPECT检查进行诊断,将术后病理学结果作为“金标准”,分析高频超声联合SPECT应用于甲状腺癌诊断中的临床价值。结果 经手术及病理证实,97例患者中良性肿瘤51例,约占52.58%;恶性肿瘤46例,约占47.42%。高频超声联合SPECT组检出良性肿瘤50例、恶性肿瘤45例,诊断准确率为97.94%;高频超声组检出良性肿瘤46例,恶性肿瘤42例,诊断准确率为90.72%。两组比较差异有统计学意义(P<0.05);高頻超声联合SPECT组无误诊情况发生,共2例漏诊,漏诊率2.06%;高频超声组1例误诊,误诊率1.03%,8例漏诊,漏诊率8.25%,差异有统计学意义(P<0.05)。结论 将高频超声、SPECT检查联合应用于甲状腺癌诊断中具有重要临床价值,可进一步提高诊断准确率,为疾病治疗工作的开展及预后评估提供可靠影像学依据。

[Abstract] Objective To investigate the clinical value of high frequency ultrasound combined with SPECT in the diagnosis of thyroid cancer. Methods A total of 97 patients with thyroid cancer admitted to our hospital from June 2017 to June 2018 were convenient selected. All patients underwent high-frequency ultrasound examination and SPECT examination for diagnosis. The pathological results after operation were regarded as "gold standard". The clinical value of high-frequency ultrasound combined with SPECT in the diagnosis of thyroid cancer was analyzed. Results Surgery and pathology confirmed that 51 cases of benign tumors accounted for 52.58% and 46 cases of malignant tumors accounted for 47.42%. Fifty cases of benign tumors and 45 cases of malignant tumors were detected by high-frequency ultrasound combined with SPECT, the diagnostic accuracy was 97.94%. And 46 cases of benign tumors and 42 cases of malignant tumors were detected by high-frequency ultrasound combined with SPECT, the diagnostic accuracy was 90.72%. The difference between the two groups was statistically significant (P<0.05). There was no misdiagnosis in the high frequency ultrasound combined with SPECT group, 2 cases were missed, the missed diagnosis rate was 2.06%. In the high frequency ultrasound group, 1 case was misdiagnosed, the misdiagnosis rate was 1.03%, 8 cases were missed, the missed diagnosis rate was 8.25%, the difference was statistically significant (P<0.05). Conclusion The combination of high frequency ultrasound and SPECT has important clinical value in the diagnosis of thyroid cancer. It can further improve the diagnostic accuracy and provide reliable imaging basis for the development of disease treatment and prognosis evaluation.
