血清NGAL在消化道息肉患者中的表达及其临床意义

【摘要】 目的 分析消化道息肉患者血清中性粒细胞明胶酶相关载脂蛋白（NGAL）的表达及其临床意义。方法 对82例消化道息肉患者（息肉组）、30例消化道肿瘤患者（肿瘤组）和27例健康者（对照组）进行血清NGAL和癌胚抗原（CEA）检测。对检测结果进行对比分析。结果 肿瘤组、息肉组、对照组血清NAGL浓度分别为（142.90±36.16）、（121.03±41.01）、（103.14±21.91）?g/ml， 肿瘤组血清NAGL浓度高于息肉组及对照组， 差异有统计学意义（P<0.05）；息肉组血清NAGL浓度高于对照组， 差异有统计学意义（P<0.05）。肿瘤组、息肉组、对照组血清CEA浓度分别为（12.39±25.54）、（2.45±1.45）、（2.32±1.79）ng/ml， 肿瘤组血清CEA浓度明显高于息肉组及对照组， 差异有统计学意义（P<0.05）；息肉组及对照组血清CEA水平比较差异无统计学意义（P>0.05）。结论 血清NGAL在消化道息肉、消化道肿瘤中表达逐渐升高， NGAL可能是筛查消化道癌前病变的潜在指标。

【Abstract】 Objective To analyze the expression of serum neutrophil gelatinase-associated lipocalin （NGAL） in patients with gastrointestinal polyps and its clinical significance. Methods Serum NGAL and carcinoembryonic antigen （CEA） were detected in 82 patients with digestive tract polyp （polyp group）， 30 patients with digestive tract tumor （tumor group） and 27 healthy subjects （control group）. Comparative analysis was made on the detection results. Results Tumor group， poly group and control group had content of serum NAGL respectively as （142.90±36.16）， （121.03±41.01） and （103.14±21.91） ?g/ml. Tumor group had higher content of serum NAGL than polyp group and control group， and the difference was statistically significant （P<0.05）. Polyp group had higher content of serum NAGL than control group， and the difference was statistically significant （P<0.05）. Tumor group， polyp group had control group had content of serum CEA respectively as （12.39±25.54）， （2.45±1.45） and （2.32±1.79） ng/ml. Tumor group had obviously higher content of serum CEA than polyp group and control group， and the difference was statistically significant （P<0.05）. Polyp group had no statistically significant difference in serum CES levels comparing with the control group （P>0.05）. Conclusion The expression of serum NGAL in digestive tract polyp and digestive tract tumor gradually increased， and NGAL might be a potential index for screening precancerous lesions of digestive tract.

中性粒細胞明胶酶相关载脂蛋白（neutrophil gelatinase-associated lipocalin， NGAL）是Lipocalin家族的新成员， 在多种恶性肿瘤中异常表达， 并与肿瘤浸润、转移、预后密切相关。目前关于NGAL与消化道息肉的相关性研究报道少见。本研究旨在探讨血清NGAL在消化道息肉患者中的表达及其临床意义。现报告如下。

1 资料与方法

1. 1 一般资料 收集2017年1～6月本院消化内科收治的82例消化道息肉患者（息肉组）、30例消化道肿瘤患者（肿瘤组）为研究对象， 均经内镜检查+病理确诊。息肉组中男44例， 女38例；年龄37～80岁；息肉类型：食管息肉2例， 胃息肉23例， 食管息肉合并胃息肉1例， 结直肠息肉47例， 胃息肉合并结肠息肉8例， 十二指肠息肉合并结肠息肉1例。肿瘤组中男16例， 女14例；年龄45～72岁；肿瘤类型：食管癌2例， 胃癌10例， 结直肠癌18例。所有患者均排除肾脏疾病、感染、严重心肺功能不全、肿瘤等疾病。选择同期本院体检健康志愿者27例为对照组， 其中男14例， 女13例；年龄30～71岁。三组性别、年龄等一般资料比较差异无统计学意义（P>0.05）， 具有可比性。本研究经兴化市人民医院伦理委员会批准， 所有受试者均知情同意。endprint
