探讨对输卵管妊娠患者采用腹腔镜保守手术治疗的临床效果及其对受孕的影响

【摘要】 目的：探讨對输卵管妊娠患者采用腹腔镜保守手术治疗的临床效果及其对受孕的影响。方法：将2010年1月-2015年9月在本院妇科治疗的120例输卵管妊娠患者随机分为对照组和观察组，各60例。对照组采用腹腔镜输卵管切除术治疗，观察组采用腹腔镜保守手术治疗，比较两组患者的临床效果、各项指标及受孕情况。结果：观察组治愈率为100%，显著高于对照组的90.00%，差异有统计学意义（P<0.05）；观察组的手术时间、术中出血量、术后镇痛时间、术后排气时间、住院时间、并发症发生率等各项指标均明显优于对照组，差异均有统计学意义（P<0.05）；观察组血hCG恢复时间、月经恢复时间均明显短于对照组，差异均有统计学意义（P<0.05）；观察组术后的双侧输卵管通畅率、输卵管不通率与对照组比较，差异均有统计学意义（P<0.05），两组通而不畅率比较差异无统计学意义（P>0.05）；观察组获得宫内妊娠率为60.00%、重复异位妊娠为13.33%，对照组分别为35.00%和6.67%，两组比较差异均有统计学意义（P<0.05）。结论：对输卵管妊娠患者采用腹腔镜保守手术治疗效果确切，对输卵管创伤小，利于术后早期恢复受孕功能，提高宫内妊娠率，具有积极的临床意义。

【Abstract】 Objective：To investigate the clinical effect of laparoscopic conservative surgery for tubal pregnancy and its effect on pregnancy.Method：120 tubal pregnancy patients in our hospital from January 2010 to September 2015 were randomly divided into the control group and the observation group，60 cases in each group.The control group was treated by laparoscopic tubal resection，the observation group was treated by laparoscopic conservative surgery，the clinical effects，indexes and the conception of the two groups were compared.Result：The cure rate of the observation group was 100%，significantly higher than 90.00% of the control group，the difference was statistically significant（P<0.05）.The operation time，intraoperative blood loss，postoperative analgesia，postoperative exhaust time，hospitalization time，incidence of complications and other indicators of the observation group were significantly better than those in control group，the differences were statistically significant（P<0.05）.The blood hCG recovery time and menstrual recovery time of the observation group were significantly shorter than those in control group，the differences were statistically significant（P<0.05）.The bilateral tubal patency rate and fallopian tube obstruction were observed after operation in observation group，which compared with the control group，the differences were statistically significant（P<0.05），there was no significant difference in the rate of fallopian tube patency of the two groups（P>0.05）.The intrauterine pregnancy rate of observation group was 60.00%，repeated ectopic pregnancy was 13.33%，the control group were 35.00% and 6.67% respectively，the differences between the two groups were statistically significant（P<0.05）.Conclusion：For tubal pregnancy patients with laparoscopic conservative surgery treatment effect is exact，the fallopian tube less trauma，is conducive to the early recovery of pregnancy function，improve the intrauterine pregnancy rate，has a positive clinical significance.endprint
