McGrath视频喉镜为强直性脊柱炎病人清醒气管插管 1例

依据近年来气管插管临床研究成果分析,各种新式的视频喉镜可能会优于传统喉镜,尤其是在处理困难气道方面[1]。我们介绍 1例应用 McGrathw视频喉镜用于严重强直性脊柱炎病人清醒下插管患者。患者,男 ,75岁,不慎跌倒,脊柱 CT提示 L2压缩性骨折,需外科手术治疗;既往严重强直性脊柱炎病史;经气道评估后(见表1),计划应用 McGrathw视频喉镜清醒下插管 ,首先,分两次将 10mg利多卡因用喷雾器对舌咽和喉咽喷雾,6min后应用 McGrathw视频喉镜行气管插管,一次成功,未见任何并发症。观察发现声门 Cormack-Lehane评分为 2级。

讨论:对于强直性脊柱炎患者,常表现为颈部僵硬和张口受限,在应用常规喉镜进行气管插管时很困难,因此,这种情况下通常使用视频喉镜。以往的研究表明,使用视频喉镜插管对于困难气道可能优于常规喉镜[2,3]。McGrathw视频喉镜和 Airway Scopew已成功的用于颈椎病和常规喉镜插管失败的患者[4]。在一项随机研究中 ,60例上呼吸道肿瘤患者应用 GlideScopew和 Airtraq进行比较,应用 GlideScopew插管的成功率为 100%,Airtraq组成功率占 93%[5]。20例强直性脊柱炎患者中,有 17例使用 GlideScopew经鼻插管获得成功。在 30例常规喉镜失败或欠佳的情况下,使用 McGrathw视频喉镜 25例插管成功,3例置管失败,2例喉镜检查失败[6]。大样本(150例)研究发现,应用 McGrathw喉镜气管插管成功率为 98%[7]。一般认为,与传统喉镜相比,应用视频喉镜频繁插管时所引起的损伤少。Copper[8]报道两例与喉镜检查有关的腭咽弓受伤,GL和 O'Leary[6]报道 4例 McGrathw视频喉镜插管期间引起局部轻微出血。总之,使用 McGrathw视频喉镜为严重的强直性脊柱炎患者清醒气管插管,简单易学,可操作性强。
