自拟益气养阴清利方治疗慢性肾炎蛋白尿的疗效观察

摘 要 目的：观察自拟益气养阴清利方治疗慢性肾炎蛋白尿的临床疗效。方法 选取2015年10月至2017年2月收治的慢性肾炎蛋白尿患者60例，按照随机数字表法分为对照组和治疗组各30例。在常规治疗基础上，对照组口服肾炎康复片治疗，每天3次，每次5片；治疗组给予自拟益气养阴清利方治疗，用水煎服，每天1剂（400 mL），分早晚2次温服；两组均治疗3个月。比较两组患者的疗效、中医证候评分、实验室检查指标及治疗期间的不良反应发生情况。结果 治疗组总有效率为87.1%（24/30），高于对照组的43.0%（13/30，P<0.05）。 治疗后，治疗组的尿液红细胞数量、24 h尿蛋白定量低于对照组，且中医证候评分改善亦优于对照组（P<0.05）。结论 治疗组的实验室指标和中医证候评分改善效果及疗效较好。

Observation of therapeutic effect of self-made Yiqi yangyin qingli recipe in the treatment of chronic nephritis proteinuria

ABSTRACT Objective： To observe the clinical effect of Yiqi yangyin qingli recipe in the treatment of chronic nephritis proteinuria. Methods： Sixty patients with chronic nephritis proteinuria treated from October 2015 to February 2017 were selected and according to the random number table method， divided into a control group and a treatment group with 30 cases in each group. On the basis of conventional treatment， the control group was treated with Shenyankangfu tablets， 3 times a day， 5 tablets each time； the treatment group was treated with self-made Yiqi yangyin qingli recipe， decocted with water， 1 dose （400 mL） a day， twice in the morning and evening； both groups were treated for 3 months. The curative effect， TCM syndrome score， laboratory test indicators and adverse reactions during the treatment period were compared between the two groups. Results： The total effective rate of the treatment group was 87.1% （24 / 30）， which was higher than that of the control group 43.0% （13 / 30 P<0.05）. After treatment， the number of urine red blood cells and 24-hour urine protein quantitative of the treatment group were lower than those of the control group， and the improvement of TCM syndrome score of the treatment group was also better than that of the control group （P<0.05）. Conclusion： The improvement effect and curative effect of laboratory indexes and TCM syndrome score of the treatment group are better.

近年來，随着我国社会经济发展及人们生活节奏日益加快，生活水平逐渐提高，慢性肾炎蛋白尿患者的发病人数呈逐年上升趋势。慢性肾炎蛋白尿患者持续存在的大量蛋白尿是慢性肾炎不良预后的危险因素。因此，对于慢性肾炎患者，阻止尿蛋白漏出是治疗的一大重点。中医学理论认为，慢性肾炎蛋白尿隶属祖国医学“石水”“虚劳” “精微外泄”范畴，主要病机特点为本虚标实证，脾肾气虚为本，风邪、湿热、瘀毒为标，湿热贯穿于病程的始终。本研究观察自拟益气养阴清利方治疗慢性肾炎蛋白尿的临床疗效，现报道如下。
