额叶癫中噘嘴发作1例

1 病例报告

患者女，23岁，慢性病程，因“反复发作性口角下撇15年余”就诊,发作时表现：发作性口角下撇，伴双眼瞪大、表情恐惧，持续时间10～20 s/次，5～6次/d。无四肢抽搐，无咂嘴、咀嚼。查体：神清，精神可。神经系统检查未见阳性定位体征。既往史、家族史无特殊。头颅MRI：未见异常。发作间期头皮脑电图：可见左侧额极、额、中央区棘波、 棘慢波散发或持续性发放；发作期头皮脑电(记录到3次发作)：清醒期，双眼睁大，表情恐惧，口角下撇，同期脑电图表现为左侧额极、额、中央区低-中波幅棘波、棘慢波、快波活动发放→广泛电压减低，持续时间约20 s(图1、2)。治疗方案：奥卡西平0.9 g，1次/早；0.6 g，1次/晚，以及开普兰0.5 g，2次/d,口服。2017-06随访:目前近半年发作1次,药物调整为奥卡西平0.3 g，2次/d,开普兰0.5 g,2次/d。
