阐述糖尿病神经病理性疼痛的发病机制

1神经性理性疼痛的概述
神经性理性疼痛是一种躯体感覺神经系统损伤或者疾病引起的疼痛。糖尿病、带状疱疹病毒、肿瘤和获得性免疫缺陷综合征等都可以导致外周神经发生损伤,当损伤持续存在较长时间后,就会产生痛觉异常或者痛觉过敏的现象。在世界范围内,我国人群的糖尿病患病率较高,约为10%,大概50%的糖尿病病人会发展成糖尿病神经病变,其中又高达25%的病人会发生糖尿病神经病理性疼痛[1-2]。
2糖尿病神经病理性疼痛的概述
糖尿病神经病理性疼痛的主要症状为麻木、疼痛和感觉异常,症状严重程度一般为中等到重度的程度,容易在夜间恶化,严重影响睡眠质量。病痛会对病人的日常生活和情绪产生负面作用,严重影响病人的生活质量。糖尿病神经病理性疼痛给病人和社会带来了沉重的经济负担[3-5]。
3糖尿病神经病理性疼痛的发病机制
在糖尿病神经病变的研究领域,科学家们一直在不断地探索。目前认为,糖尿病神经病变的发生涉及到多种途径的综合作用,这些途径包括多元醇途径、己糖胺途径、蛋白激酶C途径、晚期糖基化终末产物的聚集和能量代谢途径。这些途径的激活导致基因表达的改变、炎症和氧化应激反应以及线粒体功能的紊乱,最终导致糖尿病神经病变的发生。我们的治疗原则在高血糖血脂代谢异常的基础上得到非降脂机制,小胶质细胞,白细胞,ROS,TGF等,具体见图1[6-9]。
