探讨四肢骨折合并糖尿病的手术治疗效果

[摘要] 目的 对四肢骨折合并糖尿病患者手术治疗效果进行分析研究。方法 将该院2018年1月—2019年1月期间治疗的84例四肢骨折合并糖尿病患者作为研究对象，并对患者治疗前后血糖值情况、治疗有效率及并发症几率进行对比分析。结果 84例四肢骨折合并糖尿病患者中有65例患者痊愈，恢复良好的患者有8例，总优良率为 86.90%，其中有2例患者出现手术切口感染，2例患者出现静脉血栓的情况，总并发症几率为4.76%;治疗前，患者餐前血糖值为（11.6±0.4）mmol/L，餐后2 h血糖值为（14.6±0.3）mmol/L;治疗后，患者餐前血糖值为（6.5±0.1）mmol/L，餐后2 h血糖值为（9.5±0.2）mmol/L（P<0.05）;治疗前，患者平均生活质量评分为（5.61±0.64）分，平均疼痛程度评分为（9.16±0.23）分;治疗后，患者平均生活质量评分为（8.61±1.02）分，平均疼痛程度评分为（5.06±0.83）分（t=3.679，P=0.034;t=5.443，P=0.025）。结论 医护人员在对四肢骨折合并糖尿病患者进行手术前，应对其血糖值进行良好的控制，根据患者骨折情况选择针对性合理方式，从而使其生活质量得到有效提升。

[Abstract] Objective To analyze the effect of surgical treatment on patients with limb fracture and diabetes. Methods 84 cases of limb fractures and diabetes treated in the hospital from January 2018 to January 2019 were taken as the research object， and the blood glucose level before and after treatment， the treatment efficiency and the probability of complications were compared and analyzed. Results Of the 84 patients with extremity fractures and diabetes， 65 patients recovered， 8 patients recovered well， and the total excellent and good rate was 86.90%. Among them， 2 patients developed surgical incision infection and 2 patients developed venous thrombosis total complication rate was 4.76%; before treatment， the patient's blood glucose value before meal was （11.6±0.4）mmol/L， and the blood glucose value after meal 2 h was （14.6±0.3）mmol/L. After treatment， the patient's blood glucose value before meal（6.5±0.1）mmol/L， 2 h postprandial blood glucose value was （9.5±0.2）mmol/L（P<0.05）; before treatment， the patient s average quality of life score was （5.61±0.64）points， mean pain degree score was （9.16±0.23）points. After treatment， the patient's average quality of life score was （8.61±1.02）points， and the average pain degree score was （5.06±0.83）points（t=3.679，P=0.034; t=5.443， P=0.025）. Conclusion Before performing surgery on patients with extremity fractures and diabetes， medical staff should control their blood glucose levels and choose targeted and reasonable methods according to the fractures of patients， so as to effectively improve the quality of life.

糖尿病是较为常见的内分泌系统疾病，随着患者年龄的增长及身体机能的下降，极易发生骨折等病症，且患者完全恢复几率较小。除此之外，糖尿病患者由于血糖值处于异常状态，容易对骨骼中软组织及骨骼质量造成影响，从糖尿病患者多种并发症来看，极易出现感觉减退及视力减退等状况，从而增加了患者跌倒跌撞等风险，并提升骨折发病几率。受多方面因素影响，糖尿病合并四肢骨折患者治疗难度极大，因此，在治疗过程中，医护人员应以患者病情发展情况为依据，选取合理的治疗措施，避免术后出现不良反应[1-2]。为了使糖尿病合并四肢骨折患者治疗有效率得以提升，将该院2018年1月—2019年1月期间接受诊治的84例患者給予手术治疗，现报道如下。
