膀胱尿路上皮癌组织中生存素基因的表达及临床意义

[摘要] 目的 探討膀胱尿路上皮癌组织(BUC)中生存素(Survivin)基因的表达及临床意义。 方法 选取2010年7月~2018年12月病理科存档的BUC标本60例,采用En Vision二步法免疫组化检测Survivin基因的表达。另选取其他手术切下的正常膀胱黏膜标本20例作对照。分析BUC组织与正常膀胱黏膜组织中Survivin基因表达的差异。 结果 60例BUC中Survivin基因的表达阳性43例(71.67%)。20例正常膀胱黏膜组织中Survivin基因表达均为阴性。BUC组织中Survivin基因的表达阳性率较正常膀胱黏膜组织更高(χ2=30.99,P<0.01)。BUC组织中Survivin基因表达与患者的性别和年龄无关(P>0.05),与肿瘤是否复发、浸润深度和病理分级等密切相关(P<0.05)。结论 BUC组织中Survivin基因的表达存在明显上升趋势,Survivin基因的高表达极有可能促进BUC的发生和发展;且其表达与肿瘤的复发情况、恶性程度及病理浸润深度存在一定的相关性,可能参与BUC病情的转移和浸润的过程。

Expression and clinical significance of survivin gene in bladder urothelial carcinoma

[Abstract] Objective To investigate the expression and clinical significance of survivin gene in bladder urothelial carcinoma (BUC). Methods 60 cases of BUC specimens archived in the pathology department from July 2010 to December 2018 were selected. En Vision two-step immunohistochemistry was used to detect the expression of Survivin gene. Another 20 cases of normal bladder mucosa cut from other operations were selected as controls. The difference of Survivin gene expression between BUC tissue and normal bladder mucosa was analyzed. Results There were 43 cases (71.67%) of positive expression of Survivin gene in 60 cases of BUC. Survivin gene expression was negative in 20 normal bladder mucosa tissues.The positive rate of Survivin gene expression in BUC tissues was higher than that in normal bladder mucosa tissues(χ2=30.99, P<0.01). The expression of Survivin gene in BUC tissues was not related to gender and age of patients (P>0.05), and was closely related to tumor recurrence, depth of invasion and pathological grade(P<0.05). Conclusion The expression of Survivin gene in BUC tissue has a clear upward trend. The high expression of Survivin gene may promote the occurrence and development of BUC. The expression of Survivin gene is related to the recurrence, malignancy degree and depth of pathological invasion, and may be involved in the transfer and invasion of the BUC condition.

总之,BUC组织中Survivin基因的表达存在明显上升趋势,Survivin基因的高表达极有可能促进BUC的发生和发展;且其表达与与肿瘤的复发情况、恶性程度以及病理浸润深度存在一定的相关性,可能参与BUC病情的转移和浸润的过程。Survivin基因可作为评估术后复发风险和预后的独立指标,有望成为Survivin基因诊断及靶向治疗的新靶点,通过分子靶向治疗手段抑制BUC组织中Survivin基因而促使肿瘤细胞凋亡可能是临床治疗BUC的新途径。
