胰岛素联合饮食运动在妊娠期糖尿病控制血糖方面的研究

[摘要] 目的 探讨了胰岛素联合饮食运动在妊娠期糖尿病控制血糖方面的临床效果。 方法 方便选取2017年6月—2018年6月该院收治的妊娠期糖尿病患者87例，按照随机数表法分为两组，对照组43例，采取常规性孕期指导，观察组44例，应用胰岛素联合饮食运动进行血糖控制，将两组血糖控制效果进行对比。 结果 观察组患者血糖（7.12±1.59）mmol/L、HOMA-IR（2.72±0.74）μU/mL均优于对照组（9.21±1.28）mmol/L和（3.64±0.88）μU/mL，差异有统计学意义（t=6.744、2.282，P<0.05），观察组破宫产10例、羊水过多2例、胎儿窘迫2例、早产1例、巨大儿2例、新生儿窒息3例、妊娠期血压疾病3例少于对照组破宫产25例、羊水过多13例、胎儿窘迫10例、早产15例、巨大儿12例、新生儿窒息13例、妊娠期血压疾病12例，差异有统计学意义（χ2=11.341、10.056、6.402、15.409、8.790、7.943、6.778，P<0.05）。结论 胰岛素联合饮食运动在妊娠期糖尿病控制血糖方面，能够取得良好的应用效果。

[Abstract] Objective To investigate the clinical effect of insulin combined with diet exercise on the control of blood glucose in gestational diabetes. Methods Convenient select from June 2017 to June 2018， 87 patients with gestational diabetes mellitus admitted to our hospital were divided into two groups according to the random number table method， and 43 patients in the control group. The routine pregnancy guidance was used， and 44 patients in the observation group were treated with insulin combined diet exercise for blood glucose control， the two groups of blood glucose control effects were compared. Results The blood glucose （7.12±1.59） mmol/L and HOMA-IR （2.72±0.74） μU/mL were better in the observation group than in the control group （9.21±1.28） mmol/L and （3.64±0.88） μU/mL. Statistical significance （t=6.744， 2.282， P<0.05）， observation group 10 cases of rupture， 2 cases of amniotic fluid， 2 cases of fetal distress， 1 case of premature delivery， 2 cases of giant babies， 3 cases of neonatal asphyxia， pregnancy 3 cases of blood pressure disease were less than 25 cases of control group， 13 cases of amniotic fluid， 10 cases of fetal distress， 10 cases of premature delivery， 12 cases of premature birth， 13 cases of neonatal asphyxia， 12 cases of neonatal asphyxia， 12 cases of gestational blood pressure disease， the difference was statistical significance （χ2=11.341， 10.056， 6.402， 15.409， 8.790， 7.943， 6.778， P<0.05）. Conclusion Insulin combined with diet exercise can achieve good application results in the control of blood glucose in gestational diabetes.

妊娠期糖尿病簡称GDM多发于孕妇的妊娠期，临床症状不明显，在我国发病率在1.5%～4.3%之间[1]。近年来由于饮食结构的变换，患病人数有所上升，因此需要采取综合治疗方法控制的血糖浓度。而胰岛素联合饮食运动能够对妊娠期糖尿病具有良好控制效果[2-3]。通过运动、饮食联合胰岛素用药，能够有效调节血糖水平，改善胰岛β细胞功能，使患者的血糖水平得到有效控制。该文方便选取2017年6月—2018年6月该院收治的妊娠期糖尿病患者87例进行分组研究，报道如下。

综上所述，在妊娠期糖尿病控制血糖方面采用胰岛素联合饮食运动的控制手段，能够有效稳定孕妇体内的血糖水平，减少不良妊娠结局的发生的概率，值得临床推广。
