应用二尖瓣环自动追踪技术测量二尖瓣环收缩期位移评价左室收缩功能的准确性探讨

【摘要】目的 应用二尖瓣环自动追踪技术(AMAT)测量二尖瓣环收缩期位移,探讨AMAT評价左室收缩功能的准确性。方法 选取受检者60例作为研究对象,其中LVEF>50%者30例,LVEF<50%者30例。取心尖四腔心,分别采用M-型超声心动图法(M-型法)和AMAT法测量二尖瓣环间隔侧及侧壁处的收缩期位移(MAD-S、MAD-l);并用改良Simpsons法测定左室射血分数(LVEF)。结果 AMAT和M-型法测量的MAD-S、MAD-l无显著性差异(P>0.05);并与LVEF相关性良好(r=0.89和r=0.86)。结论 AMAT可以简便、快速、准确地测量二尖瓣环收缩期位移,为评价左室收缩功能提供了一种新方法。

【Abstract】Objective To evaluate the accuracy of automated mitral annular tracking (AMAT ) for assessing left ventricular systolic function through detemination mitral annular []displacement(MAD).Methods Sixty subjects were studied,including 30 subjects with LVEF>50% and 30 subjects with LVEF<50%,.LVEF of all subjects were determined by 2D echocardiography using Simpsons rule. MAD of septal and lateral sites of mitral annulus(MAD-S and MAD-l) from apical four-chamber view were determined by AMAT and M-mode echocardiography.Results MAD-S and MAD-l by AMAT had no significant diffrenences when compared with it measured by M-mode echocardiography(P>0.05),and correlated closely with the LVEF determined by 2D echocardiography using Simpsons rule(r=0.89和r=0.86).Conclusions AMAT can simply、rapidly and accurately detect mitral annular displacement,and provide a new technigue for evaluating left ventricular systolic function.

许多研究已经证实,二尖瓣环收缩期位移可以反映左室收缩功能的变化,而且与左室射血分数(LVEF)之间有良好相关性[1]。本研究旨在探讨AMAT法测量二尖瓣环收缩期位移评价左心室收缩功能的准确性。
1 资料与方法
1.1 一般资料
选取受检者60例作为研究对象,其中LVEF>50%者30例,LVEF<50%者30例。其中,男35例,女25例,年龄28~59岁,平均(47.8±6.9)岁。
1.2 仪器与方法
使用日本东芝公司的Toshiba Aplio XG超声诊断仪,应用改良Simpsons法测量LVEF。在心尖四腔心切面下,分别采用AMAT法和M-型法测量MAD-S、MAD-l。AMAT测量方法及曲线图见图1。以上指标均取三个心动周期的平均值。
1.3 统计学方法
采用SPSS统计学软件对数据进行处理,计量资料以“x±s”表示,采用t检验。以P<0.05为差异有统计学意义。
2 结 果
2.1 应用AMAT法和M-型法测量的二尖瓣环间隔侧及侧壁收缩期位移比较
二尖瓣环自动追踪技技术和M-型超声心动图测量的MAD-S、MAD-l比较,差异无统计学意义(P>0.05),见表1。
2.2 二尖瓣环自动追踪技术测量的MAD-S、MAD-l与LVEF具有良好相关性(r=0.89和r=0.86)
3 讨 论
研究证明,左室长轴收缩活动早于左室的短轴收缩活动,对维持正常的左室收缩功能起着重要作用[2]。收缩期二尖瓣环的运动可以直接反映左室的长轴收缩运动[3]。
M-型超声心动图测量二尖瓣环收缩期位移评价左室收缩功能是一种通用、可靠的和精确性高的超声测量方法,与二维超声心动图Simpsons法测量的EF具有良好相关性,已经得到广泛研究和认同[4];而且M-型超声心动图测量的二尖瓣环收缩期位移与核素左室造影及左室造影计算的LVEF之间有良好的相关性[5]。AMAT基于二维超声高帧频数字技术,操作简单,不受超声声束角度的影响,只需在心尖四腔心切面显示二尖瓣环间隔侧、侧壁及心尖这3个取样点,通过自动追踪左室的心内膜在整个心动周期的蛇形运动情况,计算出左心室功能及结构的各项指标[6]。本研究中AMAT法和M-型法测量二尖瓣环收缩期位移无显著性差异,表明了AMAT法作为一种新技术与M-型法测量二尖瓣环收缩期位移的一致性。而且AMAT法测量的二尖瓣环收缩期位移与LVEF之间具有良好相关性(r=0.89和r=0.86),为临床提供了客观地评价左室收缩功能的新方法。
