Research on the Application of Diversified Teaching in Gynecological Clinical Teaching

Abstract At present,the main body of clinical teaching in obstetrics and gynecology is still the traditional clinical teaching method,in which clinical teachers are the main body,and interns are the passive.It is not conducive to giving full play to interns' learning enthusiasm and ability to actively solve problems if lack of stimulating the clinical thinking and cultivating the ability to raise questions.In the face of the problems and deficiencies in gynecological clinical teaching in the new era,our team tries to adopt diversified teaching methods,such as medical record teaching method,multimedia teaching method,main-role teaching method,problem-based learning teaching method and so on,so as to improve students' ability of autonomous learning and problem-solving,as well as students' ability of innovation and cooperation.Therefore,clinical teaching teachers and teaching managers should keep pace with the times to carry out diversified teaching mode reform in gynecological clinical teaching.

Background
At present,the subject of clinical teaching in obstetrics and gynecology is still the traditional clinical teaching method,which emphasizes teaching as the basis,that is,clinical teachers teach professional knowledge.Interns only need to listen,watch and imitate.Clinical teachers are active while interns are passive.It is not conducive to giving full play to interns' learning enthusiasm and interns' ability to actively solve problems if lacks of stimulating the clinical thinking and cultivating the ability to raise questions.As a result,students generally have the problem of disconnection between specialized theory and clinical practice,which affects the quality of gynecology clinical teaching.
With the increasing awareness of patients' right to privacy protection and the increasing requirements for the quality of medical services,some gynecologic diagnosis and treatment technologies need to expose the privacy of patients,and some diseases may involve the privacy of patients,which strengthens the awareness of patients' self-protection and easily causes psychological resistance to interns.In clinical work,some interns have strong dependence,lack of communication skills,weak team spirit,poor anti-setback ability,poor self-discipline and overself,which is basically consistent with relevant reports [1].
Facing the new trends and problems of gynecological clinical practice in the new era,clinical teachers and teaching managers should carry out teaching mode reform in gynecological clinical teaching with the times in order to improve the quality of gynecological clinical teaching.It is of far-reaching significance for diversified teaching methods to replace traditional teaching methods.
Research Contents
Through diversified teaching,students should systematically master the basic theory of gynecology of traditional Chinese medicine,syndrome differentiation and treatment of traditional Chinese medicine,and master the western medicine treatment principles of common gynecological diseases.Diversified teaching methods were adopted for interns in People's Hospital of Changle County,Shandong Province.We comprehensive use multimedia classroom teaching method,experimental teaching,problem-based learning (PBL) teaching method,clinical operation training,combination of theory and practice,expert lectures and teaching,medical record discussion,leading teaching method,etc.Compared with the previous students,the passing rate of the final examination of the students increased by 89.9% and the average score of the final examination increased by 54% [2].This teaching method makes classroom teaching get better extension,especially for gynecology department with strong practicality,more attention should be paid to the combination of theoretical knowledge and clinical practice,so that classroom teaching and clinical practice form an organic whole,and continuously improve students'clinical thinking ability and innovation ability.
Research Methods
Literature methodCollect and further analyze relevant data such as relevant development background and theoretical basis through CNKI journals,relevant theoretical works of various medical disciplines and online resources,so as to better understand the current research situation and achievements on diversified teaching at home and abroad and get inspiration from them.
Interview methodFace-to-face interviews are conducted on the investigated place through pre-established open questions,materials that can objectively reflect the research object are collected through conversation with them,management problems are truly grasped,and better suggestions are put forward through analysis.
Action research methodRegularly carry out special research and topic forums in combination with the topic plan,and write research reflections,research essays,etc.
Results
Medical Record Teaching Method
Traditional Chinese medicine gynecology and western medicine gynecology have commonness,but two different systems.Gynecology is a highly specialized subject.The theoretical knowledge in books is basically abstract,which is often difficult to satisfy students' thirst for knowledge and to arouse students'enthusiasm for listening to lectures and learning.Therefore,the introduction of medical record teaching method is an effective way to solve the above problems.
Medical record teaching method [3] is a teaching method that organizes discussions around typical cases of common diseases according to the needs of teaching purposes,inspires students to make diagnosis and differential diagnosis,and puts forward treatment opinions.For example,in the chapter on ectopic pregnancy,it will enable students to keep in mind the clinical manifestations and diagnosis methods of ectopic pregnancy,and spontaneously assume a strong sense of responsibility to protect the patient's life as a doctor by citing a typical clinical case,so that students will first eliminate ectopic pregnancy and avoid life-threatening accidents when they encounter abdominal pain of childbearing age clinically in the future.
The classic medical record of each disease will connect all kinds of knowledge into a system,and make the knowledge in textbooks simple and enable students active in class and easier to master knowledge.In addition,several more cases are provided for students to analyze according to the methods taught by teachers to consolidate their knowledge [4].In the practice and attempt of subject research teaching,the advantages of medical record teaching method are more obvious than traditional teaching method,which is worthy of teaching promotion.
Multimedia Teaching Method
With the rapid development of science and technology,computers and networks have penetrated into various fields,and multimedia network teaching has been realized in major universities and teaching hospitals.The use of pictures,sounds,videos and various network resources for teaching in the form of human-computer interaction completely subverts the traditional teaching view.This teaching method is more vivid and easy to highlight the teaching focus.
Because of its strong interaction and simple operation,students' learning is not limited by time and space,so it is more popular with teachers and students.Taking uterine prolapse as an example,students can clearly remember the degree of uterine prolapse through several groups of pictures.In addition,normal anatomical maps are placed so that students can more vividly understand the pathogenesis of the disease.Multimedia teaching frees the hands of teachers and students,allows teachers sufficient time to communicate with students,and also allows students to pay more attention in class.In addition,students can copy relevant knowledge and review themselves to promote students' self-learning after class,which greatly improves the teaching quality.
Leading role teaching method
Leading role teaching [5] is a kind of teaching that emphasizes taking students as the subject and students participate in teaching as leading roles.This teaching method fully embodies students' subjectivity,actively mobilizes teaching and learning,fully exploits personal potential,and cultivates students' comprehensive intelligence,including self-study ability,clinical analysis ability,language expression ability,etc.,so that students can not only accurately understand and master knowledge,but also apply the learned knowledge to practice.
The leading-role teaching method enables students to take the initiative to receive knowledge before class.By consulting materials and expanding their knowledge,they can not only master the contents of books,but also learn more knowledge other than books,enhance their interest in learning,and at the same time have more profound memory.
In addition,students are often more interested as the teachers,and the classroom atmosphere is more active.In the process of students' lectures,teachers are often more likely to find out the difficulties that students are not easy to master,and supplement and explain them in time,which greatly improves the learning effect of students [6].
PBL teaching method
PBL teaching method is a problem-based teaching method,which uses PBL to put forward questions to students,takes students as the subject and teachers as the guide of discussion teaching method.Students can solve problems by themselves according to the basic knowledge they have learned in the past.
PBL teaching method enables students to be more impressed with knowledge points and can grasp knowledge points more firmly,thus being able to flexibly and accurately apply these knowledge points in practical problems [7].
For example,metrorrhagia,premenstruation,menorrhagia,prolonged menstruation and intermenstrual hemorrhage all have abnormal vaginal bleeding.How should they be identified?
How to distinguish deficiency from excess and how to treat breast distending pain according to syndrome differentiation.
Teachers use similar methods of contrast and inspiration to make students think and discuss,and to promote students to learn independent learning and integrated learning.
When PBL teaching method is introduced into teaching,students' ability to analyze problems,logical reasoning,expression,communication and information access to obtain knowledge have been trained and improved.
Clinical teaching method
Gynecology with integrated traditional Chinese and western medicine is a practical discipline.Practice is the only criterion for testing truth.In order to strengthen students' clinical practice ability,students'regular clinical practice teaching mode is adopted in teaching.In the teaching of gynecology combined with traditional Chinese and western medicine,the knowledge imparting method of simple injection teaching is no longer applicable.Let students participate in clinical work regularly,not only can students put the knowledge they have learned into practice,but also is conducive to doctor-patient communication and cultivate good medical ethics [8].
Gynecological patients are all women,and the diseases they suffer from are reproductive system diseases.Both medical history collection and physical examination involve privacy.In the current harsh medical environment,the patients' awareness of privacy protection has increased.Whether the patients have resistance or the students themselves are afraid of their hands and feet,it has brought difficulties to the internship.Male students will have more troubles in gynecology clinic.Before contacting patients,let students make full use of simulated people to carry out practical operations and master operation skills skillfully.Students should be supercilious in front of patients.The teacher fully respects the students and calls them doctors so that the students can establish prestige in front of the patients.
Conclusion
Compared with the traditional teaching mode,the diversified teaching method has fundamental differences in design concept,implementation mode,evaluation system and actual effect.Diversified teaching method is characterized by carrying out the teaching idea of "taking students as the center".Emphasis is placed on the teaching idea of setting up doubts,questioning and solving doubts based on problems,which aims to provide students with an environment that promotes learning.Students are encouraged to take the initiative to ask questions and analyze them together in group discussions.It is emphasized that learning should be set into the problem situation,so that students can learn the specialized theoretical knowledge hidden behind the problem by analyzing the authenticity problem,form the skills to solve the problem,and cultivate the ability of autonomous learning,so that students can use various resources to find gaps in information and knowledge,and cooperate with each other to solve the problem.
Diversified teaching emphasizes the combination of basic theory and practical ability,thoroughly changes learners' passive learning methods,and highlights learning initiative and the formation process of knowledge and skills.Diversified teaching helps students develop key skills of autonomous learning,communication and problem solving,which helps students to have a deeper understanding of professional theoretical knowledge and gives full play to the internal potential of their major.
