Clinical experience of Professor Huang Yahui on the same treatment for different diseases by applying the method of invigorating spleen and resolving phlegm

Abstract

Introduction

Professor Huang Yahui has recognized that spleen deficiency and phlegm dampness are the key triggers and common pathogenesis of related diseases, based on the following theories: “various diseases occur due to internal injuries on the spleen and stomach”, “various diseases are derived from phlegm”, “various diseases are affected by phlegm”, and spleen is the source of generating phlegm.Professor Huang applies the method of invigorating spleen and resolving phlegm in treatment of various clinical diseases and has obtained remarkable effects.

TCM Mechanism of the Same Treatment for Different Diseases Using the Method of Invigorating Spleen and Resolving Phlegm

In the Piwei Lun (Treatise on Spleen and Stomach),human’s life is derived from the yang qi, which is generated from the spleen and stomach; human’s life is also developed through the yin qi, which is also generated from the spleen and stomach; Yin essence contributes to the long life, and it originates from the spleen and stomach; food and drink contribute to nourish human spirit, and they transport and transform in the spleen and stomach.No damage to the spleen and stomach can ensure that the body has sufficient vitality.Once the spleen and stomach are naturally weak and damaged by external factors such as improper or dirty diet, the vitality cannot nourish the internal organs, accordingly lead to various diseases.Therefore, spleen replenishment is the premise of strengthening human energy[1].According to TCM theory, the phlegm may cause all kinds of diseases,and phlegm-fluid retention may occur in the spleen and stomach, any deficiency in the spleen and stomach may affect the transportation and transformation of water and dampness, accordingly leading to phlegm-fluid retention,which blocks the qi and flows through the whole body,causing a series of diseases[2-3].Therefore, the occurrence and development of the diseases are a pathological process dominated by spleen deficiency and phlegm dampness as a important pathogenic factor.

Thoughts on Syndrome-Differentiation Diagnosis and Treatment

Emphasizing on spleen and stomach, generating constant source

The spleen and stomach are the foundation of acquired constitution, the source of qi and blood, and the essence of the five internal organs.Qi and blood are one of the important substances for maintaining the human vitality,the blood is constantly derived from the essence of water and food, but it is actually generated and transformed in the spleen, as Zhang Jingyue proposed; In the Zhangshi Yitong (Medicine Writings of Zhang’s Family) written by Zhang Lu, the spleen is the source of qi, indicating that qi and blood originate from the spleen and stomach and are born from the water and food, and function to nourish the internal organs[4].In the Yizong Bidu (Essential Medical Book) written by Li Zhongzi in Ming Dynasty, once the human body is generated, it should be fed with food essence, the food enters the stomach and is distributed to the organs, then the blood is generated and human body develops and survives[5].

Zhang Zhongjing proposes that, the spleen is prosperous and is not affected in all seasons; Li Dongyuan believes that, sufficient vitality in the human body is necessary to prevent the occurrence of disease, and the vitality qi is born in the spleen and stomach; If there is no internal factor causing spleen and stomach weakness, it cannot trigger the occurrence of disease even though there are external evils, that is, the evil alone can’t damage people and diseases are derived from the spleen and stomach[6].Therefore, clinical treatment of diseases should emphasize the principle of protecting the spleen and stomach as the core, only when the spleen and stomach are under normal function, the qi and blood essence can be constantly generated like endless water source.Therefore,in treatment of any disease, strengthening the spleen can regulate the five internal organs, and harmonize blood and qi.

Expelling phlegm dampness, avoiding spleen disturbance

In the Linzheng Zhinan Yi’an (Guide to Clinical Practice with Medical Records), the stomach is naturally dry and the spleen is naturally wet, the stomach prefers to be wet and the spleen prefers to be dry, these two organs are interdependent and integrating Yin and Yang, they contribute to complete the transfer of food and drink[7].If the spleen and stomach of the body are naturally weak,any endogenous dampness evil or exogenous dampness evil or the combination of evils can further disturb and damage the spleen, causing the dysfunctions of spleen and stomach to transport water, food and water retention.In course of time, it develops into pathological products such as phlegm, retention and stasis, causes other visceral lesions and refractory protracted illness.Phlegm dampness is not only a pathological product of spleen deficiency, but also a pathological factor leading to other diseases, they are closely related clinically.Therefore, it’s suggested to expel phlegm dampness, avoid spleen disturbance, ensure normal functions of the spleen and stomach, thus cut offthe pathway of further deterioration of the diseases.

Regulating qi activity, avoiding stagnation

In the Suwen · Liu Weizhi Dalun (Plain Questions), qi dysfunction in going out and going away from the body will destroy the spiritual mechanism, while qi dysfunction in rising and falling will make qi at risk alone.So without qi going and travelling, human body can’t grow and survive; without qi rising and falling, human body can’t have ecologic changes[8].This evidence emphasizes the importance of qi to human living activities, and smooth qi activity mainly depends on the spleen (stomach), liver and lungs.First, the spleen acts as the central soil, which belongs to Taiyin and functions to ascending, the stomach acts as the Yang soil, which belongs to Yangming and functions to descending.The two organs are placed in the middle jiao, as is the hub of qi activity.They regulate the rise and fall of qi in the five internal organs, and maintain normal living activities.Secondly, Ye Tianshi proposes that, the liver is the trigger of the diseases and the stomach is the transmitter of the diseases, any liver disease must be committed to the soil and is the result of the evils,the spleen disturbance causes abdominal distension.In the Suwen · Baoming Quanxing Lun (Plain Question),the soil and the wood are interconnected, which points out that the liver and spleen interact with each other.Smoothing the liver can help the spleen to transport essence, and dispersing the spleen qi can help the liver to rise.The qi activity can be regulated by the harmonization of liver and spleen, dispersing liver wood will regulate the qi-blood in spleen soil.Finally, in the Yixue Shizaiyi(Brief on TCM Principle-Method-Recipe-Medicines),qi travels in the lung channels, and all qi in the internal organs and meridians are declared by the lung qi; The turbidity-reducing function of the stomach is completed by the purifying and descending function of the lungs; the lungs and the large intestine are interior-exteriorly related,the conduction function of large intestine depends on the purifying and descending function of the lungs; qi in the whole body is attributed to the lungs.When the lung qi is cured, it is easy for qi travelling and dispersing stagnation,so the lung treatment is necessary.Therefore, dispersing lung qi, harmonizing the lung-spleen and liver-lung may invigorate the spleen and harmonize the stomach, lead to the liver ascending and the lung descending, transport the helm, so the qi, blood and body fluid are reconciled[9].

Examples of Medical Records

Chronic atrophic gastritis with erosion

Liu**, male, 60 years old.On June 28, 2018, he was admitted to the hospital due to intermittent fullness and discomfort in stomach cavity for 2 years, and worsening with anorexia for 2 months.Two years ago, stomach cavity fullness occurred due to irregular diet, which was slight or serious, and he was treated poorly by the local hospital.Within past 2 months, the gastric cavity fullness became worsened due to inadvertent diet, accompanied with anorexia, odor in the mouth, pale tongue, slightly thick white tongue, and fine pulse.Gastroscopy finding: fungal esophagitis? Chronic atrophic gastritis with erosion;pyloric disease detection: moderate chronic inflammation of small pyloric mucosal tissue, activity (++), atrophy(+), intestinal metaplasia (+).HP: (-).Western medicine diagnosis: 1.Chronic atrophic gastritis with erosion; 2.Fungal esophagitis.TCM diagnosis: distention and fullness of stomach.Syndrome differentiation: spleen deficiency and phlegm obstruction.Treatment: strengthening the spleen and expelling phlegm, harmonizing the stomach and eliminate stomach distention.Prescription: orange peel 12 g, Pinellia Rhizoma prepared by ginger 12 g,Poria 15 g, honey-fried licorice root 6 g, atractylodes 10 g, Magnolia officinalis 10 g, rhizoma bletillae 10 g,Astragalus 30 g, curcuma zedoary 10 g, radish seed 15 g,radix curcuma 10 g, Scutellaria barbata 15 g, coix seed 30 g, Sophora 10 g.7 doses, in water decoction, twice a day.Second visit: after medication, fullness and distention, as well as odor in the mouth were relieved by half, appetite was improved, with pale tongue, white tongue, deep thin pulse.Scutellaria barbata, atractylodes and raw coix seed in the above prescription were deleted.Supplements in the above prescription: Fructus Aurantii 15 g, raw atractylodes 20 g, Dendrobium 15 g, bergamot 15 g, chicken’s Gizzardmembrane 15 g.After 2-week medication, all syndromes disappeared.During 1-year follow-ups, the fullness and distension were not reported.

Analysis: It has been confirmed that one of the development paths of gastric cancer is: superficial gastritis→ atrophic gastritis → intestinal metaplasia → atypical dysplasia → gastric cancer[10].Therefore, active treatment of atrophic gastritis can block the path of gastric cancer development to a certain extent.Professor Huang Yahui believes that, this patient is suffering from damage to the spleen and stomach due to improper diet, then dysfunction of transportation, internal phlegm dampness,and poor qi activity in the middle jiao lead to stomach distention and fullness.Phlegm dampness blockage,depression suppressing spleen yang, poor transportation of spleen qi and stomach qi, can cause anorexia; phlegm dampness and long-term depression develop the heat,dampness heat accumulates in the mouth, so odor in the mouth occurs; pale tongue, slightly thick and white coated tongue, fine pulse are the syndromes of spleen deficiency and phlegm obstruction.The treatment should focus on strengthening the spleen and expelling the phlegm, once the spleen is strengthened, the phlegm is not generated; once the phlegm is eliminated, the spleen is not disturbed.Fangzhong Chenpi Decoction can dry dampness and strengthen spleen, eliminate phlegm and benefit qi, in addition, Radix Astragali strengthens the spleen and nourishes qi; radish seed helps digestion,eliminates phlegm and descends qi; coix seed strengthens the spleen and eliminates dampness; curcuma aromatica smoothes the liver, regulates qi and expels stasis; based on the modern pharmacological research, rhizoma bletillae protects membrane and relieves hyperacidity, restrains sore and promotes granulation; curcuma zedoary and Scutellaria barbata promote blood circulation, remove stagnation, relieve internal toxin and prevent cancer;Sophora flavescens clears heat, dries dampness, and antiinflammatory.These four medicines are effective for diseases[11-13].At the second visit, the above symptoms were significantly relieved.Scutellaria barbata and coix seed were deleted to prevent cold on the stomach, and Atractylodes was deleted to prevent yin damage.Fructus Aurantii was added to descend qi, raw atractylodes was added to strengthen spleen and benefit qi, Dendrobium was added to nourish yin, tonify stomach and clear heat,bergamot was added to dispel depression and promote qi circulation, chicken’s Gizzard-membrane was added to help digestion and harmonize stomach.The above herbs were added or subtracted according to the symptoms, after medications all symptoms were alleviated.

Acute attack of chronic atrophic gastritis

Wang**, female, 36 years old.On October 16, 2017, she firstly visited the hospital due to recurring hiccups for over 1 year.The patient had repeated hiccups over the past year,and no systemic treatment was given.Current symptoms:frequent hiccups, oppression under the xiphoid process,occasional acid regurgitation, dry and bitter mouth,stomach cavity fullness after eating, stool forming, once or twice/day, occasional desire to defecate and difficulty in bowel movement, good appetite, poor night rest, normal urination.Self-complaint of impatient temperament.Red tip of the tongue, thick and white coated tongue, string and thin pulse.Gastroscopy finding: chronic atrophic gastritis with acute inflammation.HP (-).Western medicine diagnosis: Acute attack of chronic atrophic gastritis.TCM diagnosis: hiccups.Syndrome differentiation: liver stagnation and spleen deficiency, phlegm obstruction and qi reversal.Treatment principle: strengthening the spleen and resolving phlegm, soothing the liver and harmonizing the stomach, calming the reversed energy and stop hiccup.Prescriptions: Bamboo Shavings 12 g, Pinellia ternate 12 g, Poria 15 g, immature orange fruit 15 g, orange peel 12 g,honey-fried licorice root 6 g, Bupleurum 10 g, Scutellaria baicalensis 10 g, Radix Pseudostellariae 15g, Bindweed 12 g, reddle 15 g, radish seed 15 g, fried chicken’s Gizzardmembrane 15 g, Sichuan lovage rhizome 10 g, parched white peony root 15 g, roasted malt 15 g, Divine Comedia 15 g.7 doses, in water decoction, twice a day.Second visit:hiccups and oppression under the xiphoid process were relieved by 70%, with occasional stomach cavity pains.Sichuan lovage rhizome and parched white peony root in the above prescription were deleted.Supplement in the prescription: dried rehmannia root 15 g, tree peony root bark 10 g, rhizome bletillae 10 g, for another 7 doses.Third visit: the above symptoms basically disappeared, and the oral prescription was maintained and given for 2 weeks,then all symptoms disappeared.During 1-year follow-up,no hiccups occurred.

Analysis: Professor Huang Yahui advocates the principle of“differentiating the disease as the initial and differentiating the syndromes as the basis” in the clinic, the treatment should be formulated based on the disease and syndromes.The patient usually has impatience, liver dysfunction,insult in spleen, dysfunction of spleen in transportation,internal dampness and turbidity, accumulation in the central soil, suppression on stomach yang, and liver qi invading the stomach, stomach qi rising, diaphragm qi depression, uprushing the throat, thus leading to hiccups; phlegm dampness and stagnation, qi depression in the middle jiao may lead to the depression under the xiphoid process; long-term liver qi stagnation develops heat and invades stomach, causing acid regurgitation;red tongue tip, thick and white coated tongue, string and fine pulse were the syndromes of liver stagnation and spleen deficiency, phlegm obstruction and qi reversal.The etiology and pathogenesis of hiccups are mostly related to coldness in the lungs and stomach, causing the stomach qi to move towards the diaphragm.In the Jingyue Quanshu ·Hiccups (Jing-yue’s Complete Works), the hiccup is always caused by qi reversal.The underneath reversed flow of qi may move upwards, without qi, there is no hiccup, without yang, there is also no hiccup, the source of the hiccups must be qi[14].Fangzhong Xiaochaihu Decoction can smooth the liver and promote qi circulation, strengthen the spleen and stomach, Hewendan Decoction can regulate qi, eliminate phlegm and harmonize stomach.Inula flower and riddle were added to resolve phlegm,calm the adverse energy and prevent hiccup; radish seeds, chicken’s Gizzard-membrane, malt and Divina Commedia can promote digestion and qi circulation,expel phlegm; Rhizome of Chuanxiong promote qi-blood circulation; white peony root and Bupleurum falcatum can disperse stagnated liver, relieve depression and promote qi circulation.At the second visit, the symptoms were significantly relieved.Sichuan lovage rhizome and parched white peony root were deleted.Raw rehmannia root 15 g and tree peony root bark 10 g were added to nourish yin and reinforce stomach; rhizome bletillae 10 g was added to protect membrane and relieve hyperacidity.A longterm medication in a low dosage eventually strengthen the spleen and eliminated phlegm, smoothed the liver and treated the chronic disease.

Acute attack of chronic bronchitis

Li**, female, 76 years old.On July 25, 2018, she firstly visited the hospital due to intermittent cough and expectoration for over 10 years, worsening within 1 month.Ten years ago, she was diagnosed with bronchitis upon cough and expectoration, and received intermittent treatment, but the disease condition was poorly controlled.The symptoms of cough and expectoration recurred and aggravated 1 month ago, coughing a lot of white foam sputum and worsening upon the odor, accompanied by itching, occasional chest distress, palpitation and shortness of breath, these symptoms became obvious after doing activity, in addition she had occasional acid regurgitation, acceptable appetite and rest, shapeless stool,once or twice/day, normal urination.Self-complaint of fearing cold.Pale tongue, thick and white coated tongue,string and thin pulse.The patient had a previous history of bronchitis for 10 years and coronary heart disease for 6 years.She had undergone stent placement in the First Affiliated Hospital of Xi’an Jiaotong University, and then orally took Gelan Ningxin Capsule, Shexiang Baoxin Pill, Rosuvastatin Calcium Tablet, the disease condition was generally controlled; she also had a 5-year history of hypertension, with blood pressure up to 160/100 mmHg,and intermittently orally took Ramipril as antihypertensive treatment, but blood pressure was not monitored regularly.Western medicine diagnosis: 1.Acute attack of chronic bronchitis; 2.Coronary atherosclerotic heart disease,with cardiac function grade II; 3.Hypertension grade-2(high-risk group).TCM diagnosis: cough.Syndrome differentiation: qi deficiency of lungs and spleen, internal stagnation of phlegm dampness.Treatment principle:tonifying the lungs and strengthening the spleen, expelling phlegm and relieving cough.Prescription: orange peel 12 g, Rhizoma Pinelliae 12 g, Poria 15 g, licorice root 6 g, Atractylodes 10 g, Magnolia officinalis 10 g, fried coix seed 30 g, perilla stem 15 g, Platycodon grandiflorum 10 g, white hyacinth bean 15 g, aster 12 g, radix stemonae 12 g, dried ginger 6 g, fructus amomi 6 g, Cynanchum glaucescens 12 g, calcined oyster shell 30 g, forsythia 15 g.7 doses, in water decoction, twice a day.Second visit:cough and expectoration were relieved by half, with dry mouth and sometimes bitter mouth.Cynanchum glaucescens in the above prescription was deleted.Supplement: Bupleurum 10 g, white peony root 15 g, for another 7 doses.Third visit: the above symptoms basically disappeared, and the prescription was effective and maintained unchanged for subsequent 3 weeks, and all the symptoms disappeared.During 1-year follow-up, no cough and expectoration were found.

Analysis: Cough was firstly reported in the Neijing (Endo-Meridians), Suwen · Cough Theory (Plain Questions): all internal organs may lead to the coughing, rather than the lung alone.This points out that, the cough lesion is mainly in the lungs and is related to the liver, spleen, kidneys and other organs.Professor Huang Yahui believes that,this patient is old and weak, and a long-term medication may damage the spleen and stomach, then the internal phlegm develops and move upwards the lungs, the lungs lose their function of purifying and descending, so the cough occurs.Chronic cough damages the lungs (qi), and the qi cannot digest the distention, so phlegm dampness are more likely to form, and the disease condition is stretching and refractory; phlegm dampness and obstruction disturb the mind, so chest distress, palpitation and shortness of breath occur; pale tongue, thick and white coated tongue, deep thin pulse were the syndromes of lung and spleen qi deficiency, internal stagnation of phlegm dampness.Fangzhong Erchen Decoction can dry dampness and relieve phlegm, regulate qi and regulate the middle warmer; Hepingwei San can dry dampness and transport spleen, promote qi circulation and harmonize the stomach; fried coix seed and white hyacinth bean may strengthen the spleen and eliminate dampness; fructus amomi strengthens the spleen, promote qi circulation and regulate the middle warmer; perilla stem regulates vital energy and widens the middle warmer; platycodon grandiflorum reinforces the lungs and relieves phlegm;Cynanchum glaucescens descends the lungs and resolves phlegm, appropriate purifying and descending may help to regulate the qi activity; Aster moistens the lungs,depresses qi and relieves cough; dried ginger warms the lungs and resolves phlegm, in the Jingui Yaolue (Synopsis of Golden Chamber) written by Zhang Zhongjing, phlegm and retained fluid should be treated with warm herbs[15];calcined oyster shell relieves hyperacidity and pain; fructus forsythia clears heat and detoxifies.At the second visit,the symptoms were obviously relieved.Then Cynanchum glaucescens was deleted, and the prescription was added with 10 g of Bupleurum to relieve depression and promote qi circulation, with 15 g of white peony root to nourish blood and restrain yin.By using the classical prescription with simple herbs, the disease was cured.

Hypoproteinemia

Liu**, male, 67 years old.On November 19, 2018, he firstly visited the hospital due to abdominal distending pain and discomfort accompanied with physical fatigue for half a year.Half a year ago, he felt abdominal fullness and discomfort due to irregular diet, and received no standard treatment.Afterwards, the symptoms gradually aggravated, accompanied by fatigue and chilling.Then he was hospitalized in a local hospital.Conventional blood test showed: NETU% 79.8%, RBC 2.63 × 1012/L, HGB 73 g/L, HCT 24.4%; B-ultrasound of upper abdomen showed: enhanced echo of liver parenchyma,intrahepatic calcification, enlarged spleen, and peritoneal effusion; heart color ultrasound showed: full right heart, mild regurgitation of atrioventricular valve and aortic valve, reduced left ventricular diastolic function,pericardial effusion; liver function showed: TBIL 27.4 mmol/L, ALT 298 U/L, AKP 358 U/L, ALB 27.10 g/L,Tp 58.9 g/L.After symptomatic treatment, his symptoms were eased and he was discharged.Current symptoms:abdominal fullness, occasional abdominal pain, dry and bitter mouth, belching, fatigue, chilling, shortness of breath, occasional palpitation, general appetite, good night rest, dry stool, once every 2 days, unsmooth bowel movements and endless defecation feeling, normal urination.Pale red tongue, thick and white coated tongue, string and thin pulse.In 2014, he has undergone cholecystectomy due to cholecystolithiasis and received intraoperative blood transfusion.Western medicine diagnosis: 1.Hypoproteinemia; 2.Moderate anemia; 3.Splenomegaly.TCM diagnosis: abdominal retention.Syndrome differentiation: spleen deficiency, dampness accumulation, and liver depression.Treatment principle:warming the body, strengthening the spleen, smoothing the liver, regulating vital energy, dispersing dampness,and promoting diuresis.Prescription: Bupleurum 10 g, Scutellaria baicalensis 10 g, Rhizoma Pinellia 12 g,Codonopsis pilosula 15 g, Atractylodes 10 g, Magnolia officinalis 10 g, orange peel 12 g, Fructus Aurantii Immaturus 15 g, Astragalus membranaceus 30 g, Angelica 20 g, dried rehmannia root 30 g, plantain 15 g, coix seed 30 g, white peony root 15 g, rhizome of chuanxiong 10 g, pericarpium arecae 15 g.7 doses, in water decoction,twice a day.Second visit: Abdominal distension, dry and bitter mouth was relieved by 60%.He still felt abdominal pain, the fatigue and chilling were improved, the belching,palpitation and shortness of breath basically disappeared.The appetite was improved.The stools were slightly dry, once every 2 days.Unsmooth bowel movements and endless defecation feeling were relieved.The above prescription was added with 15 g of rhizome corydalis, for another 7 doses.Third visit: the symptoms were alleviated obviously, Codonopsis pilosula was deleted and Astragalus was reduced to 20 g, cardamom 10 g and lophatherum gracile 12 g were added, for another 7 doses.Fourth visit:all syndromes basically disappeared, and the prescription was effective and maintained unchanged.The medication was orally taken for 2 weeks.During the follow-up for half a year, no abdominal retention was found.
